CONSENT TO TREATMENT

Welcome! This document outlines the important details regarding the psychological services I offer. Please read this information carefully and ask any questions you may have before signing below. By signing this form, you acknowledge that you have read and understood it and have had all your questions answered. If you use insurance benefits, as described in the section below called Insurance Reimbursement, then by signing this form you are also giving me permission to share your information with your insurance company.

About Me

I am Rebecca Hatton, PsyD, a fully licensed psychologist in the state of Michigan.

My license number is 6301012318.

Services Offered

I provide psychotherapy primarily for adults who have unusual experiences. This may include hearing voices that others do not, spiritual experiences or spiritual emergency, having strong fears that disrupt ordinary life, or having a sense of receiving special messages. I also provide psychotherapy for depression, anxiety, and loneliness.

I have advanced training in several approaches to assisting people who have a diagnosis of ‘schizophrenia’ and related conditions. I have been trained both by professionals with expertise in recovery-oriented psychotherapy for psychosis and by people recovered from their own personal psychological emergencies. I utilize ideas from:

Open Dialogue family therapy, with therapy team

Cognitive behavioral therapy for psychosis (CBTp)

Hearing Voices Network self-help (Romme et al., 2009)

Dialogue with voices (Ron Coleman, 2018)

Maastricht Interviews for Voices/Paranoia (Pete Bullimore, nationalparanoianetwork.org)

Mindfulness-based approaches, such as Compassion-Focused and Acceptance and Commitment Therapy for psychosis (ACTp)

Most of my services are “individual”, meaning I usually work one-to-one. I can occasionally provide dialogic family therapy as well.

About Open Dialogue

When fully implemented, Open Dialogue team-and-family response to psychiatric crisis has superior outcomes. Dialogic therapists work as a team, inviting and welcoming any family members, friends, clergy, or other supports the person in crisis wishes to have present. Dialogic therapy is not available in full in Michigan, however.

I have 400 in-person hours of training and several years’ experience using elements of Open Dialogue. OD processes that I am generally unable to provide include: immediate help, daily family therapy, in-home services, 24/7 support, or services fully-funded by national health care. So, I need to be able to collaborate with other support services. Family members, other providers involved with the person in crisis, and emergency services are sometimes needed in the course of dialogic therapy.

I reserve the right to decline to provide services that, in my view, do not have sufficient safety elements to ensure your welfare. I may not be able to provide adequate frequency of therapy meetings or crisis services or to collaborate as needed with your other providers and supportive family members.

Services Not Offered

I do not provide treatment that is involuntary, medication management, ADHD assessment, couples therapy, substance abuse counseling, or dialectical behavior therapy. If areas in which I do not practice appear to be the primary problem, I will refer you to counselors with appropriate expertise.

Psychotherapy

Therapy is a collaborative process that requires your active participation. I will listen closely to you as you determine your goals and direction and strive to follow your preferences. The process of therapy, however, often involves discussing uncomfortable topics. There are no guarantees about the outcome of therapy, but extensive research has shown that a majority of people who engage in the therapies listed above find them beneficial.

Benefits and Risks of Psychotherapy

Therapy requires a significant investment of time, energy, and sometimes money. Therapy can be a helpful and effective way to gain acceptance of and address unusual mental experiences, difficult thoughts or memories, and troubling feelings. However, therapy can also involve emotional discomfort, uncomfortable realizations, and uncertainty about how to proceed. Throughout any therapy sessions, I encourage you to ask questions. Also, feel free to seek a second opinion at any time.

Confidentiality

All information discussed in therapy sessions will be kept confidential, unless you give me written permission to share such information. I respect your confidentiality and will follow your wishes regarding privacy unless I believe you or others might come to harm. If mental distress becomes so urgent I believe outside support is needed, I will do everything I can to encourage you to seek that help yourself. This may include requesting evaluation for inpatient treatment or reporting an unsafe situation to emergency services.

There are exceptions to your right to confidentiality, including:

Alternatives to my Services

If you have experienced unusual mental states or distress for less than two years, you may be eligible to receive an array of evidence-based services at the Program for Risk Evaluation and Prevention (PREP) program in the University of Michigan’s Department of Psychiatry. For further information: Psych-PREP@med.umich.edu

The Early Treatment and Cognitive Health (ETCH) group assists with early psychosis in E. Lansing, MI. Like U of M’s PREP program, this is a highly experienced collaborative offering evidence-based services. Go to etchwellness.org for further information.

Your local Community Mental Health agency is an excellent resource for psychiatric care and social work support. CMH clinics often offer support and social groups, referral to vocational rehabilitation, housing support, in-home check-ins if needed, assistance obtaining Social Security or health benefits, 24-hour crisis assistance, and other essential care. Find access information by searching online or calling 211 information.

Rose Hill Center in Holly, MI provides residential treatment in a farm setting. Rose Hill is a non-profit that can charge private insurers, but not Medicare or Medicaid. It has some limited scholarships, mostly reserved for referrals from a CMH clinic. For further information: rosehillcenter.org

The Therapist Directory at psychologytoday.com offers a comprehensive list of psychiatric, psychology, and social work providers. You can enter search terms for location, type of service, specialty, insurance, etc.

Fees

My standard fee for a therapy session is $210 for 55 minute session. My standard fee for a 90 minute dialogic family meeting is $225. If I am empaneled by your insurance, I accept the hourly fee for a 90 minute family session. Family therapy may include a teammate, whose fee is in addition to mine. I accept insurance and co-pays as full payment from several insurers, including straight Medicare, most Blue Cross plans, Blue Care Network, Blue Cross Complete Medicaid, Health Alliance Plan, some Medicare Advantage plans, and others. At this time (2026) most insurers also cover phone conversations 20 to 55 minutes long and telehealth (such as meetings on zoom), in addition to in-person therapy. If you have Medicare and Medicaid, I am allowed to bill you for the annual deductible, but am not permitted to bill for the 20% Medicare co-pay. All other insurances require me to bill for the full deductible and co-pay amounts unless you let me know that this would be a hardship. I often reduce or waive fees that are not affordable for a client or family.

Insurance does not pay for missed sessions or for time you are not present if you are late. It does not pay for time spent writing reports, record-keeping, consultations with other professionals, preparing medical records or treatment summaries (for disability evaluations, for example), or work on legal matters. I pay a biller to submit paperwork to your insurer. I generally do not charge for or pass these costs on to clients. There may be circumstances, however, if the work becomes very time-consuming, in which I will bill you at my standard hourly rate. I will inform you ahead of time, before I do the work, and get your prior permission. I am not allowed by law to bill you more than $400 per year over and above agreed-upon fees and do not pass on surprise costs to clients if that would be a hardship.

If you lose insurance or face major financial reversals during therapy, I do not stop working with you unless there is another appropriate provider available to you. Please bring the issue to my attention so we can work out a solution.

Payment and Attendance Policy

I am strongly committed to making my profession accessible and affordable for people who don’t fall into the well-insured or 1% groups. I never want my fees to be a hardship for therapy partners. I will continue to make many allowances. These are my payment policies:

Fees, deductibles, and co-pays are payable with cash or check. I do not have credit card or Venmo capacity. PayPal access has a 3% surcharge. I bill via email on a monthly or bimonthly basis. Payments are due at the time of service or when billed.

Insurance Reimbursement

If you choose to use insurance, please be aware that you are responsible for any copay, coinsurance, or deductible associated with your plan. If any therapy fees will be a hardship, please discuss it with me.

Your insurer may limit the number of sessions that are covered by insurance. Should you request more sessions beyond your insurance coverage, you would be responsible for additional session fees that we agree upon ahead of time.

I recommend contacting your insurance company directly and in advance of our first session to understand your specific mental health coverage benefits and any limitations or pre-authorization requirements.

Most insurance companies, including Medicare and Medicaid, require a diagnosis to provide coverage and may request additional clinical information (treatment plans, progress notes, etc.). When you sign this form, you are giving me permission to share your information with your insurance company to seek payment for your covered services.

Choosing not to use your insurance for some or all your care. You have the right to pay for services yourself to avoid these limitations and potential privacy concerns associated with using your insurance.
